Please Note: Texas Heart Institute — the Denton A. Cooley Building is located adjacent to CHI St. Luke’s Health - Baylor St. Luke’s Medical Center at the Texas Medical Center (TMC), near the intersection of Fannin Street and Holcombe Boulevard. Entrance to the Cooley Building is only through Baylor St. Luke's Medical Center Bates Street lobby. The Denton A. Cooley auditorium is located on the lower level of the Cooley Building accessible by the Green Elevators.

The Certified Healthcare Facility Manager (CHFM) program offered by the American Hospital Association Certification Center is one way that you can validate your knowledge in the profession and gain further recognition among your peers. The CHFM certification exam is based on practical experience and knowledge you may have gained academically as well as on the job. While there are many ways that you may chose to prepare for the exam, the CHFM Certification Preparation Workshop review course is one of those ways. Although participation in the review class does not guarantee that you will pass the exam, it is one way that you may chose to prepare for the exam and to validate your knowledge of content covered on the exam.

ASHE has developed an educational program intended to aid CHFM candidates in their preparation for the exam. The program is a one day program, presented by an ASHE faculty member, which presents material from the five subject areas of the CHFM exam. This course is intended to assist the candidate in their preparation by augmenting their knowledge of the fundamental skills needed to be a healthcare facility manager. The preparation course is representative of material on the exam but does not teach to the exam.

Through a combination of lecture, CHFM formatted practice test questions, and study materials, you will gain confidence in the five key competency areas of the CHFM exam. Learn what you resources you need to review, and how to take a computer-based exam.

TAHFM is partnering with the American Hospital Association Certification Center (AHA-CC) to offer a special administration of the Certified Healthcare Facility Manager (CHFM) certification examination in San Antonio. Eligible candidates may want to take advantage of this opportunity to earn the nationally recognized CHFM credential. Register early to ensure a seat for the exam.

The CHFM certification examination is based on practical experience and knowledge that you may have gained academically as well as on the job. Candidates must meet eligibility requirements fully to complete the CHFM examination. Eligible candidates who receive a passing score on the CHFM certification exam earn the CHFM certification. For more information about the CHFM eligibility requirements, exam content outline and exam administration policy, refer to the CHFM Candidate Handbook.

Examination Date: August 7, 2015

Registration Deadline:
• AMP must receive the exam application and fees by the deadline of Thursday, July 16, 2015.    
• Late applications will not be accepted. There is no onsite registration for the exam.

NOTE: The exam administration begins after all registered candidates have checked in or 9:00 am, whichever occurs first. Candidates are allowed two (2) hours to complete the exam after administrative matters are completed. Schedule your travel arrangements accordingly.

Exam registration and fees are not included with conference registration or any pre-conference sessions. Exam application and fee must be submitted to AMP as directed on the exam application.
Member fee for CHFM exam: $275
Non-member fee for CHFM exam: $425

Exam Application
• Applications cannot be submitted online for this special administration. They must be submitted to AMP by fax or mail.
• Obtain an exam application from the CHFM Candidate Handbook OR download the exam application
• If you are using the exam application in the CHFM Candidate Handbook for Examination Type, check "Special domestic administration” and write in August 7, 2015 - Houston, TX.
• Include payment with the application. AMP does not accept personal checks.
Submit exam application and payment to AMP for receipt by AMP July 16, 2015. (AMP is the vendor with which the AHA-CC contracts for exam administration.) By submitting the CHFM Examination, you certify that you meet the CHFM eligibility requirements as listed on the application and in the CHFM Candidate Handbook.

Exam Registration Confirmation
• Candidates are not registered for the exam until they have received their confirmation letter.
• AMP sends confirmation of receipt of the exam application (via mail and e-mail . If you do not receive a confirmation by about July 20, 2015, contact AMP at 1.888.519.9901 or info@goAMP.com

Day of the Exam
All policies and procedures as listed in the CHFM Candidate Handbook apply.
• Only candidates who received an exam confirmation from AMP will be allowed to test. Candidates who did not advance register will NOT be allowed to test at the conference.
• Candidates must be on time. Registration/check in begins at 8:0
